Buyer Hotel Reimbursement Online

The Chicago Collective will reimburse you $50 per night in any one of the hotels listed below.  This offer is valid only for the nights of August 10, 11, 12, 2012.

In order to be eligible for reimbursement, you must make your own hotel reservation through MMPI Travel Services:  Toll Free: 1-800-528-8700 or online at www.chicagocollectiveonline.com.

 

  1. Reimbursement is valid only for the hotels listed below. Please be sure to make your reservation BEFORE submitting this form.
  2. In order to receive a reimbursement check you must fill out this form completely. You will receive a confirmation number via email from Kerri Hannen once your form has been received and processed.  Please allow 72 hours.
  3. Reimbursement checks will be mailed after the Show.

You will receive a confirmation from The Chicago Collective once your submission has been received. Please call 312-527-7759 before July 20, 2012 if you have submitted your form, but have not received confirmation from us.
